Affordable child immunization clinics are offered through IDPH Vaccines for Children (VFC) Program each week at the Hoffman Estates Village Hall. Immunizations are for eligible children 2 months to 18 years. Appointments are necessary. All communities are welcome.
Cost is $20 for each vaccine for both residents and non-residents, or free with Medicaid.
Reservations are required. Please follow the steps below:
- To schedule an appointment, complete the VFC Eligibility and Child Screening Form through the new portal: https://portal.hoffmanestates.org/Vaccine/ChildVaccine
- Upload your child's Immunization Records where indicated--you may also take a picture of your child's immunization records and text to (847) 474-9396 or email to HHS@vohe.org
- Upload your child's Medicaid card or proof of underinsured status.
Once eligibility is confirmed, a nurse will call you to discuss vaccines and schedule an appointment.
If you cancel, change or miss your scheduled appointment with less than 24-hour notice, Hoffman Estates reserves the right to charge a $25.00 out-of-pocket fee.
